PHREAK - Netscape builds in telephony.

There's also a MAC version, despite the source of the following.

It's free, but then there's this "Distribution Authorization Form" that's
sort of creepy, which asks, "Do you agree not to export PGPfone in
violation of the export control laws of the United States of America as
implemented by the United States Department of State Office of Defense
Trade Controls?"  

Of course to use it for international "calls" you'd have to export it. 
Illegally.  Interestingly, MIT's ftp server guesses whether you are in 
the U.S. or not, or at least tries.
