Subjective: Pt. reports she is unable to concentrate: Ideas are half-formed and never quite reach an actionable level.
Objective: pt. is inarticulate although not aphasic; her explanations are baroque and full of tangents leading nowhere
Assessment: premature conceptual secretions
Plan: Rewire the cerebellum-mouth connection to prevent mouth from operating without participation by the brain

Pts complain of feeling detached from self and body as if watching themselves from a distance or in a dream. Some also report anesthesia, loss of self-control, difficulty speaking, and feelings of derealization.
